Does anyone know what a documented mental health issue is?

I was diagnosed with depression 6 years ago, and in the family court report to the judge the mediator stated I have a documented mental health issue, I have no clue to what that would be or what would fallow under that category, does anyone know.and if the mediator or anyone else for that matter did research wouldn't I have to of signed a medical records release or something?How can they find this all edged documented mental health issue If I cant?
